Continuum of Care
The CoC program is permanent supportive housing where Grace House holds the lease and is responsible for the utilities, and the tenant has a housing agreement with the organization. Tenants assisted through this housing type have HIV, Chemical Dependency, or another HUD-recognized disability. This housing is reserved for people experiencing homelessness, prioritizing chronically homeless persons. Chronically, homeless persons have been homeless for over a year consecutively or four times or more in the past three years, totaling 12 months of homelessness. Tenants pay 30% of their adjusted monthly income to Grace House, which returns the money to services provided through this program. Our CoC Program includes the following housing options:
GRACE HOUSE FOR WOMEN
Part of our CoC program, Grace House for Women, provides housing and supportive services to chemically dependent homeless women. Persons assisted in this program are required to maintain sobriety to ensure the safety and well-being of self and co-participants. Suppose someone in the program fails to maintain sobriety. In that case, every effort is made to provide additional support to help address sobriety issues to maintain tenancy. Likewise, suppose removal from the program is necessary to preserve the safety and well-being of others. In that case, every effort will be made to ensure continued housing through another Grace House or community partner program.
Scattered-site Housing
In addition to GHW scattered-site, permanent housing is also provided through Master Leases under the CoC program for households who are chronically homeless and have a household member with a qualifying diagnosis.
